Client Rights & Complaints

New Leaf Counseling Houston is the private practice of Francisco "Frank" Chapa, a Licensed Professional Counselor in Texas (License #87158) and in Colorado (License LPC.0024412).

If you ever have a concern about the services you receive here, I hope you will bring it to me directly so that we can work through it together. You also have the right, at any time, to file a complaint with the board that licenses me in your state.

FILING A COMPLAINT IN TEXAS

Licensed professional counselors in Texas are regulated by the Texas Behavioral Health Executive Council.

The Texas Behavioral Health Executive Council investigates and prosecutes professional misconduct committed by marriage and family therapists, professional counselors, psychologists, psychological associates, social workers, and licensed specialists in school psychology.

Although not every complaint against or dispute with a licensee involves professional misconduct, the Executive Council will provide you with information about how to file a complaint.

FILING A COMPLAINT IN COLORADO

The practice of licensed or registered persons in the field of psychotherapy is regulated by the Mental Health Licensing Section of the Division of Professions and Occupations. The Colorado State Board of Licensed Professional Counselor Examiners can be reached at 1560 Broadway, Suite 1350, Denver, Colorado 80202, (303) 894-7800.

Colorado clients also receive a written Mandatory Disclosure Statement before services begin, which describes these rights in more detail.

YOUR RIGHT TO A GOOD FAITH ESTIMATE

You have the right to receive a "Good Faith Estimate" explaining how much your care will cost.

Under the law, health care providers need to give patients who don't have insurance or who are not using insurance an estimate of the bill for medical items and services.

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services.

Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.

If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

Make sure to save a copy or picture of your Good Faith Estimate.
